About Premier Supported Living
Premier Supported Living, formerly known as Premier Disability Care Services (PDCS),
stands as Victoria's foremost transitional support provider. Our passion lies in aiding individuals living
with disabilities to discover their forever homes and empowering them through our supported living
services. From 2022 to 2023, we successfully transitioned 51 individuals across Victoria from
hospitals, forensic care units, and aged care facilities.
With decades of experience spanning the aged care, disability, and remote community sectors, our
founders have honed the best solutions to support NDIS participants with specific transitional
requirements. Through our programs and partnerships, we deliver tangible outcomes for young
people transitioning from residential aged care and hospital discharge settings.
Our expertise extends beyond immediate accommodation and supported living solutions. We
specialize in developing assessment reports and care plans to facilitate ongoing funding applications
with the NDIS. As leaders in transition and support services, we are committed to understanding and
addressing the needs of those we support.

Transition
We specialise in immediate transition and accommodation, offering three options for transition with our health and aged care partners
Hybrid Funding*
Hospital-funded rental costs for up to three months, with NDIS-funded care.
MTA Funding
NDIS-funded rental and support costs for a limited time period.
Affordable Disability Accommodation
A funding model designed for participants with limited financial resources.
Our Accommodation
Our accommodation services cater to diverse needs:
Short Term Accommodation (STA)
Available for up to 28 days per plan year.
Medium-Term Accommodation (MTA)
Available for up to 90 days per year, with potential for extension.
Supported Independent Living (SIL)
Ongoing support in an apartment or group home setting.
Specialist Disability Accommodation (SDA)
Modified accessibility living environments.
